HEIF is the underlying container format; HEIC is the specific file extension Apple uses for HEIF images, typically encoded with HEVC. In practice, "HEIC file" and "HEIF photo" refer to the same thing on an iPhone.
Will the JPG look different from the original?
It should look extremely similar at normal viewing sizes, but it's a new lossy encoding, not a lossless format conversion. Repeatedly converting back and forth will degrade quality slightly each time, same as any JPG re-save.
What happens to photo metadata like location and date?
This tool focuses on the image data itself; camera/location/date metadata isn't guaranteed to carry over the same way it does when AirDropping or exporting from Photos. If you need that metadata preserved exactly, check the output before deleting your original.
